All information on your child and his/her family will be held in strictest confidence. Files will be annually updated and reviewed by parents. The file will contain medical and developmental history forms, authorization and consent forms, progress and conference reports. You have the right to review your child’s file. Although you may not take it, we can supply you with duplications upon written request and a small payment for the cost of copying.
As a parent, you have the right to add information, comments, data or other relevant material to your child’s record. You also have the right to request deletion or amendment of any information contained in the child’s record.
At the discretion of a state licensor or a staff member your child’s records maybe reviewed. This would be done to ensure that the center has followed their requirements in collecting and filing the proper information. All information in the record is kept confidential, and the person reviewing the file will complete the log sheet record.
Upon written consent and within two business days after the initial request, a staff member will transfer a copy of the child’s record to the parent or any other person the parent designates.
A staff member will notify parents if their child’s records have been subpoenaed.
